Your Office Visit

After being scheduled for a new patient appointment, you will be seen as soon as possible. Be prepared to bring anything that will help us provide you the best treatment; including medical records, lab tests, and a list of all your current medications. We ask that you bring a copy of your insurance card and a photo ID to every visit. Please arrive 15 minutes before your appointment, unless instructed otherwise.

Appointments

Appointments can be made by calling our office during normal business hours, Monday through Friday, 8am-4:30pm
We attempt to stay on schedule to keep you from waiting. Please do not be late for appointments. We appreciate your patience for the occasional emergency which may delay your visit. If you cannot keep an appointment, please let us know immediately so others can be seen and keep our wait time to a minimum.  A $25.00 fee MAY be assessed for a missed appointment.

Phone Calls and Prescription Refills

Phone calls will be returned by the nurse as soon as possible.
We request you first verify with your pharmacist that there are no more refills available. Have your pharmacy phone number available when you call.  Because we need your chart to refill most medications, routine refills will not be phoned in after hours or on weekends. Prescriptions are renewed during normal office hours. Please give 24 hours notice for all prescription requests. In many instances, we will require that a patient be seen in our office prior to medication renewal.

After Hours & Emergency Care

If you are experiencing a life-threatening emergency, dial 911 or go to the nearest emergency room.  For established patients, OB and GYN emergency care is available after hours.  On-call physicians are available 24 hours a day 7 days a week.
Please use our after hours numbers for OB/GYN related emergencies only.  If an after hours call is deemed non-emergent, a fee of $25 MAY be billed to you.

Financial

All co-payments and deductibles are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards. Please contact our office at (205) 838-3036 and request Billing should you have any financial questions or concerns.

Administrative Service Fees

Due to the increase in the amount of forms for employers, insurances, and schools, a fee will be charged to cover the physician and staff’s time to review your medical record and complete your request.  The initial Family Medical Leave Act (FMLA) form is $25 and any subsequent changes or additional forms are $15.  School forms, physical forms, and immunization records are $25.

Routine medical records requests are handled daily by a dedicated staff member.  Please allow up to 30 days to process your medical records request.  To have your records request expedited, please complete a medical records request form and submit it to the office.  A search fee of $5, the cost of postage, and $1 per page for the first 25 pages, and 50¢ per page thereafter will be billed to the patient and/or requesting company.
